LAVC Individualized Service Plan For A Client Presentation

Section 1 For this section, you will need to identify who your client is and the services they are seeking. For example, are you helping a client from a lower socioeconomic background who is seeking assistance with food security? If so, what resources do you need to get them in contact with? This portion will inform section 2, as the service that your client is seeking is the system you will be researching. In the case example above, it says the client is looking for assistance with food security, therefore you would most likely get them connected with Temporary Assistance for Needy Families (TANF) or the Food Stamp Program (FSP). What will this section look like? You will be creating a mock action plan by making an Individualized Service Plans (ISPs) for your client. Don’t worry! We are not making an official one, but rather a brief outline that resembles information collected for an ISP For guidance on this section please look at 4.8 lectures notes here You must answer the following questions: Who is your client? What is the final goal you are aiming to achieve? (This is where you would list the service they are seeking) What are actionable steps you can take to achieve this goal? What resources do you they have access to?  Does this plan need to be updated regularly?              Section 2 This portion of the presentation includes an analysis and evaluation of the following service you listed above in your client’s case. For example, Temporary Assistance for Needy Families (TANF), the Food Stamp Program (FSP), Supplemental Security Income (SSI), housing assistance, Medicaid, Medicare and the Earned Income Tax Credit (EITC) You CAN choose one not listed, BUT you must let me know what your selection is so I can approve it. For your analysis you will: Introduce this service to me as if I have never heard of it. What does this service do? How long has it been around? Who or what founded this service? Was it a president? A club? Who is eligible? What does the application process look like? Who does this service help? In other words what are the demographics of the population that use this service. NOTE: This should be supported by statistical data, NOT opinion based. For your evaluation you will: In this portion I want you to use your own personal insight to highlight the pros and cons of the program. For example, do you think this program helps the population it seeks to? Can people realistically access this program with ease? Once applied and approved for the program is it difficult to remain in the program? What does the program offer? Are people satisfied with the services offered? Are you satisfied with the program Why or why not?  How can it be improved? What are its shortcomings? What do you believe may have been different about those individuals who obeyed until the very end and those who refused to go on and who stopped delivering the shocks? In other words what personality traits, attitudes or other factors do you believe were different about those who stopped and those who went all the way. Were these findings surprising to you and what do you believe this study reveals (if anything) about human nature?
