Mental Health Nursing Care Plan – Choose 2 mental health disorders, other than delirium, dementia, and personality disorders, discussed in this module and complete the template below for each chosen disorder. Each care plan is worth 50 points.

Choose a nursing diagnosis and build by filling in the template. For example: Risk for violence: Self-directed or other directed.

Fill out the columns as follows:

• List a minimum of 3 risk factors
• List a minimum of 3 defining characteristics
• List outcome criteria for each defining characteristic
• List at least one long-term goal for the client
• List at least 2 short-term goals for the client
• List at least 3 interventions (pharmacological and/or non-pharmacological)
• Provide a rational for each intervention
