Assessment Worker Scenarios
Please read the followingt w o case scenarios and write your plan of intervention.
1. The reporteris an Occupational Therapist with theInfant and Toddlers Program. Reporter goes out
to the home 2 times amonth. An eight year oldchild resides with mother and fatherand three siblings: twins (3 months old) and a 3 year old.
According to the Reporter, today when she arriveda t the home, the 8year old answered the door. Reporter statedthat child told herthat her mom wasn’t home and no other adults were there. Child reported that mom wentt o thestore to. by”nursery water” and invited the reported inside. Reporter advised that all four children were in the home. Reporter stated that all ofthe children werefine. Reporter advised that she was at theh o m e for “maybe5 minutes” and mom arrived. Mom reported that she went toToys RUs to purchase “nurserywater” as she realized she was out of it and neededt o feed the babies. Reporter stated that she told mom she might be making a report.

2. Reporter called to report Ms.K gave birth to BabyGirl at UMMS Baltimore on 4/10/2018. Both
mother and child tested positive for Marijuana. Ms. Kreported using marijuana during her pregnancy due to her being in a”stressful” time, citing homelessness and other stressors as whyshe used. Baby Girl was born 5 lbs 11.2 oz, 39 weeks 4days, Apgar of 8 and 8 . Ms. Kreports she is residing with her mother, name unknown at 18 MD Ave, Cambridge, MD 21613. Reported stated Ms. Kwas”not happy” about the referral being madeand did not provide much informationregardingher druguse or history. Ms. Kwas transferred to UMMS Baltimore due ot her having ahigh risk pregnancy due toa short cervix,and preeclampsia. Medical staff reported due to medical concerns the “baby had to come” and Ms. Kgave birth. There were no other concerns regarding Ms. K’s ability to care for the child.Ms. Kreported having ample supplies for Baby Girl at her mother’s home in Cambridge. The father’s name isMr. Kand his contact, demographic information is unknown. His involvement is unknown as well.The reporter has requested a callfromthe assigned worker to coordinate discharge and planning for the child. Family is scheduled to bedischarged as early asThursday, February 12, 2018. Reporterscontact number is410- 328-9722.
