Assignment Information

For this assignment, you have two tasks 1) identify a standardized scale to be used in your change project and 2) create an Individualized Rating Scale (IRS) to be used for your change project. 

Your standardized scale should be used at least twice: at the start and at the end of the two-week intervention. Your IRS will be used to collect at least 8 baseline data points prior to the start of your intervention and daily data points during your two-week intervention (~10-14 total intervention phase data points).

For this assignment, you will use the form that begins on the next page of this document. Write in your responses under each heading. Be sure to fully answer each section with enough detail that your responses fully convey your understanding of the process. Copy and paste the form information into a new document. Bullet points and single-spacing are expected. Include an APA formatted Cover Page and Reference Page. When referencing information about the standardized scale, include an in-text citation for all sources used (websites, articles, and/or books). 

Logistics of Completion

Note: The standardized instrument must be completed prior to the start of the intervention (baseline) and again at the end of the intervention (intervention). The IRS must be completed at least 8 times prior to the start of the intervention (baseline) and then daily during the intervention (intervention).
