DQ TURP

Discussion Question:

A 68-year-old male patient is admitted to the medical/surgical unit after having a TURP.  He has continual bladder irrigation running. Upon assessing the urine output, the nurse notes red urine with multiple large, quarter to half-dollar-sized clots. The patient’s vitals are 104/46, T: 100.4 oral, P: 89, and regular, R: 22.

  • What priority assessments and nursing interventions are needed for this patient?
  • Support/explain your thought processes.
